Steps Required to File a Claim

  • Download the claim form that is found here: HH_Claim_Form.pdf
  • Email your completed claim form to: claims@chipmanrelo.com
  • All claims must be filed within 30 days from date of delivery.
  • We'll need to know:
    1. Damaged Area
    2. Where it took place – Pick Up or Delivery Address
    3. Repair Amount (an estimation is okay)
    4. PHOTOS – If unable to upload, you can provide later
  • DO NOT perform any repairs
